We're glad you're back! If you would like to schedule an appointment online through our Patient Portal, you will need your username and password you received during one of your previous visits. Once logged in, select the button that reads "Book New Visit" and once you have selected your appointment, select the "Book It" button.
If your insurance information has changed since your last visit, please contact the office or send us a secure message through the Patient Portal to update your medical and vision insurance. If we have any issues verifying your insurance someone will reach out to you.
