Available Locations

  • Choose from one of the two buttons above to be redirected to our online scheduler. There are two separate schedulers for the Madison and Middleton Locations.

  • After you schedule an appointment, please complete our registration forms by selecting the blue button that says "View Appointment." Completing this form will take care of some preliminary questions and save time during your appointment.

  • If you are unable to complete the registration forms while booking your appointment, you will receive a text prior to your appointment with a link to the form.

  • If you have any questions regarding the schedule or registration form, please contact us for assistance.

What to bring to your Appointment:

  • Insurance cards

  • Medication List

  • Current Glasses (Even if they are broken or you mainly wear contacts)

  • ​​​​​​​Sunglasses (Prescription or non-prescription sunglasses are encouraged for everyone to protect your eyes from sun damage)

  • New Patients: Current contact lens information (e.g. contact boxes, blister packs, or written prescription)
